What Is the Usage of Uterine Hemostasis Balloon?

Three lumen two balloon tube has long been used in the emergency treatment of esophageal fundus variceal bleeding caused by portal hypertension. Three lumen refers to the gastric tube and the pipeline leading to the gastric balloon and esophageal balloon respectively, and two balloons refers to the gastric balloon and esophageal balloon. After applying a large amount of liquid paraffin, the Three lumen two balloon tube was gently placed into the stomach through the nasal cavity. First, inflate 250 ~ 300ml air into the gastric balloon with a syringe, and clamp its orifice with vascular forceps to prevent gas escape. Then pull the three lumen tube outward. When you feel moderate elastic resistance, it means that the gastric air sac has been pressed at the bottom of the stomach. Then fix it on the bed corner frame with a 0.5kg heavy sandbag or saline bottle through a pulley for continuous traction, so as to compress the bleeding vein at the bottom of the stomach. After observation, if the hemostasis is still not compressed, 100 ~ 200ml of air can be injected into the esophageal sac, and then the mouth of the tube can be clamped to directly compress the dilated vein of the lower esophagus. After the three chamber and two capsule tube is placed, the gastric contents can be pumped from the gastric tube regularly to observe whether there is continuous bleeding, and nasal feeding and related treatment can be carried out from the gastric tube.

Another common example of balloon compression hemostasis is the treatment of postpartum hemorrhage. For postpartum hemorrhage due to uterine asthenia, strengthening uterine contraction is the most rapid and effective hemostasis method, including massaging the uterus and applying uterine contractions (oxytocin, prostaglandin), etc. when the above treatment is ineffective, in order to preserve the uterus or postpartum hemorrhage, uterine cavity filling and compression hemostasis can be used. In the past, the commonly used method was uterine cavity gauze filling and compression hemostasis, However, "gauze legacy" has been perplexing medical staff, which is also one of the reasons for medical disputes. Balloon uterine stents can now be used instead of gauze. The principle of balloon hemostasis is to produce a hydrostatic pressure from the uterine cavity to the outside of the uterine cavity (the maximum water injection volume can be 500ml), which is greater than the uterine artery pressure to achieve the purpose of compression hemostasis.

American company cook medical and domestic SCW Medicath Ltd. first developed balloon uterine stent for the treatment of postpartum hemorrhage foreign and domestic respectively. There are also reports of using balloons to treat biliary bleeding and respiratory bleeding, which will not be described in detail here.
