When to use endotracheal intubation or tracheotomy?

Endotracheal intubation and tracheotomy are first-aid measures and means to assist breathing or keep the airway unobstructed. The preparation time for endotracheal intubation is shorter, the operation process is faster, and there are fewer complications in the corresponding operation process. It is more suitable for all kinds of general anesthesia operations, including airway obstruction and emergency situations in which critical patients need temporary rescue. Endotracheal intubation is mostly inserted into the endotracheal tube through the glottis from the mouth, and some endotracheal intubation through the nose. Tracheotomy is a kind of surgical operation. Select the incision in front of the neck, cut it layer by layer, separate the subcutaneous tissue, lift the trachea, and make a shaped incision between the two trachea cartilage rings (mostly at the cartilage of the third and fourth trachea in the middle of the neck), insert the trachea cannula into the main airway and keep the air passage open. The trachea cannula can be connected with a ventilator to support treatment, and it is also convenient for sputum suction and management. During the operation of tracheotomy, pay attention to the possible injury to the anterior cervical blood vessels and thyroid gland, and pay attention to hemostasis during the operation.

Generally speaking, endotracheal intubation is simple, easy and convenient, but the indwelling time of endotracheal intubation generally does not exceed one week. If the indwelling time exceeds one week or for a long time, tracheotomy is required. Endotracheal intubation should not be used in the following cases: laryngeal edema, acute inflammation of trachea, abscess of throat, compression of trachea by thoracic aortic aneurysm, fracture of cervical spine, etc.

Laryngeal obstruction, neck compression (bleeding after thyroidectomy), many lower respiratory tract secretions, unable to cough up by themselves, requiring long-term mechanical ventilation and facilitating some head and neck operations, tracheotomy should be selected. If necessary, the patient can wear a tube and leave the hospital. The disposable tracheotomy intubation kit made of high polymer can be used for short-term use. For patients who use it for a long time or leave the hospital with a tube, the tracheotomy intubation kit made of stainless steel can be used or replaced. Tracheotomy and cannula placement may have the following complications: subcutaneous emphysema, pneumothorax, mediastinal emphysema, bleeding, acute pulmonary edema, respiratory arrest, cannula prolapse, tracheoesophageal fistula, difficulty in extubation, laryngeal stenosis, etc.
